Where/Whom Will I Have To Contact?
In the UK you will have to write to the examination bodies concerned.
They usually have application forms for this. A lot of old examination boards have been merged or taken over, so check first.
There usually exists a fee (usually between £20-30) per certificate for changing the certificates.
In some cases replacement certificates will not be available, instead you may receive notification of results.
What Qualifications Does This Apply To?
In general it should be possible to change all qualifications/examination certificates.
However, some bodies may refuse, such as Universities.
If this happens then you should write an appeal to their examination office with supporting evidence included, such as your Statutory Declaration/Deed Poll and GP/Psychiatrist’s letter.
If they again refuse, write to the LGBT (or equivelant) at the University requesting their help with this matter, their support will be helpful should you have to take your appeal to the University Senate Body. However such steps would be rare.

Write directly to the Data Management Team, including ORIGINAL copy of your name change document, and a letter from a doctor stating you have gender dysphoria, or have undergone gender reassignment, enclosing payment (current cost is £25.50), your original birth certificate and your original exam certificates if you have them. The exam certificates are just to make the searches for your qualifications easier, and speed up the processing of replacement certificates.
